Note (1): (a) For GE's SupremeHealth Plan, the deductible applicable for policyholders after the age of 85 will be raised by $1,000 of the above listed deductible.

(c)For AIA Plans, the deductible applicable for policyholders after the age of 82 will be raised by 150% of the listed deductibles.

(e)For NTUC IncomeShield Preferred Plan/Plan P, the deductible applicable for policyholders after the age of 80 is as follows: Class C - $2,000, Class B2 - $3,000, Class B1 - $3,000, Class A and pte hosp - $4,500, Day Surgery - $4,500

(f)For Aviva's MyShield Plan, the deductible applicable for policyholders after the age of 80 will be 150% of the listed deductibles.

Note (2): Final Expense Benefit refers to the waiver of the deductible and co-insurance up to the stipulated amounts in the event of the insured's death during hospitalisation or within a stipulated period after discharge from hospital subject to the conditions as stipulated by the insurer.

Note (3): AIA's extra cover for Cancer and 30 Critical Illnesses Benefit and Aviva's 5 Critical Illnesses Benefit is provided over and above the Overall Policy Year Limit and the Overall Lifetime Limit. Note (4): (a) GE's SupremeHealth Plan P Plus premiums listed are rounded up to the nearest dollar and include a 5% premium discount.

(b) For PruShield A Premier, foreigners not related to Singaporeans and PRs will be subjected to higher premium rates

Note (5): The recipient's kidney or liver failure must be first diagnosed after 2 years from the commencement/ reinstatement/ last upgrading date of the policy.

For GE and Aviva plans, the living organ donor transplant applies for kidney and liver transplants only, where the recipient must be a family member (parents, siblings, children or spouse) of the Life Insured.

Note (6): Expenses incurred by the non-insured living donor shall only be reimbursed under the Living Organ Donor Benefit if the living donor is not eligible to be reimbursed under MediShield, Medisave-approved Integrated Shield Plans or any other insurance plans for the organ donation.

Note (7): Aviva's free coverage of child(ren) below 20 years old is only limited to the first four children of members with effect from 1st September 2010. An insured person who is already insured under this benefit will continue to be covered for free subject to the terms and conditions of this benefit.

The premiums payable are based on your age next birthday and may increase as members enters into the next age band The annual Medisave withdrawal limits for Medisave-approved Integrated Shield policies are:

(a)$800 per policy year, where the insured person is less than 81 years of age at his next birthday on date of policy commencement/renewal

(b)$1,150 per policy year, where the insured person is 81 or more years of age at his next birthday on date of policy commencement/renewal
